If a company account does not exist with us, please provide us with relevant contact details, so we
can send an account to the appropriate organisation, department and person
If it’s your first visit to our clinic, you will be asked to complete a Patient Registration Form
When with the doctor...
Inform your doctor at the very beginning of appointment if you:
Have any allergies e.g. eggs, antibiotics, mercury etc
Are pregnant
Have been unwell in the last 48 to 72 hours
Suffer from any medical problems which affect immunity e.g. leukaemia, HIV etc
Are on any medications which might cause immunosuppression e.g. prednisolone
Suffer from any bleeding disorders
Have history of fainting when receiving vaccination
At the beginning of the appointment, be specific about:
Any medications you are currently taking and medical conditions you suffer from
Other issues you feel might be relevant (e.g. stress, anxiety, relationship difficulties etc)
Ask the doctor to clarify for you anything which is unclear
Raise any questions you may have for the doctor
